How To Begin Pediatric In-Home Care Through GAPP

Initial Call & Home
Care Assessment
We start with a simple phone call to learn about your child’s medical needs, daily routines, and current supports. During this first conversation, our team explains how the Georgia Pediatric Program (GAPP) works, reviews basic GAPP eligibility criteria, and answers your questions.
If it appears your child may qualify, we schedule a visit with one of our licensed nurses for a detailed in-home assessment and to gather the information needed for your child’s care plan.

Physician Plan of
Treatment Approval
After the in-home assessment, our team organizes the clinical information gathered during the visit and provides it to your child’s primary care physician.
Once the physician reviews the information, they then submit a PPOT to GAPP. The program reviews all documentation and determines the number of Medicaid-covered nursing hours your child is approved to receive.

Caregiver Match &
Ongoing Support
Once GAPP approval is received, we carefully match your family with a pediatric nurse or qualified caregiver who has experience supporting medically fragile children and your child’s specific needs. We introduce caregivers before placement so the fit feels right for your family.
After services begin, a supervising RN provides regular check-ins, updates the care plan as needed, and coordinates with your child’s physician and our team to ensure your child continues to receive high-quality pediatric in-home nursing care in Georgia.

Skilled Pediatric Nursing (GAPP-Approved Care)
Our licensed pediatric nurses provide skilled, clinically-directed care for medically fragile children, following your child’s Physician’s Plan of Treatment (PPOT) and the standards outlined by the Georgia Pediatric Program (GAPP). Our skilled nursing services support children with trachs, vents, feeding tubes, seizure disorders, developmental disabilities, and other medically complex needs across Georgia.
Skilled Nursing Services Include:

Medication administration & documentation

G-tube / J-tube feeding management, site care, troubleshooting

Pediatric tracheostomy care, suctioning, and airway support

Ventilator and respiratory monitoring (oxygen, CPT, vest treatments)

Seizure monitoring and emergency response protocols

Skilled observation, assessments, and vital signs charting

Infection control, safety monitoring, and home medical oversight

Clinical communication with your child’s physician and the GAPP clinical review team

Supportive Daily Living Care (Non-Skilled Tasks)
These are non-skilled tasks that support your child’s comfort, safety, and development. They are performed by a caregiver or nurse within their scope of practice and only as outlined in the PPOT. These essential care tasks help medically fragile children remain safe, stable, and comfortable at home while receiving GAPP-approved nursing services in Georgia.
Supportive Care Includes:

Bathing, grooming, oral care, and personal hygiene

Dressing and safe repositioning for medically fragile children

Transfers and mobility assistance (bed → wheelchair, adaptive seating)

Support with sensory, motor, or adaptive feeding needs

Assistance with therapy-directed exercises (ROM, gait training, standers)

Encouraging age-appropriate activities and independence

Routine monitoring for safety, comfort, and developmental engagement

Non-skilled tasks performed in alignment with the Physician’s Plan of Treatment (PPOT)

Care Coordination & GAPP Navigation for Families
We work alongside your child’s physician and therapy team to make the Georgia Pediatric Program (GAPP) process as smooth and stress-free as possible — from the first phone call to ongoing care.
